Goniosynechialysis GSL, physically separates peripheral anterior synechiae (PAS) from the trabecular meshwork using a spatula or microforceps under direct visualization with a goniolens. It is an effective option to treat mild to moderate PACD in conjunction with cataract surgery. Optimized visualization is the key for success of GSL. In case of phaco-GSL, phaco in advance or later relies on the surgeon’s choice. However, the optimized visualization stand point, performing the gonio surgery in advance is recommended, unless there are any problems.
