LENS REMOVAL AND ENDOSCOPY ASSISTED VITRECTOMY FOR MALIGNANT GLAUCOMA
Malignant glaucoma can be induced sometimes after trabeculectomy especially in “hot” uncontrolled IOP eyes. Mechanisms are not fully understood but there are some commonly agreed hypothesis including: ciliary – lens contact, tears in anterior hyaloid with valve effect. My philosophy in management for those cases are lens removal (+/- IOL) – anterior vitrectomy – hyaloid-zonulo-iridetomy for balancing pressure in front and behind the iris reforming the anterior chamber.
